Program Requirements:
- Have a business plan
- Have an Income Statement and Balance Sheet of the last 2 years
- Business tax returns of the last 2 years
- If registered as a legal entity, certificate is needed
- Must be owner/co-owner
- Resume of owner/co-owner
- Certificate of formation
- W-9 form
- IRS letter with EIN number
- Business tax receipt (for current and previous year)
- Proof of business location
- Proof of Citizen
- Copy of driver license
- Article of Incorporation
